Allstate Workplace Division's Group Voluntary Products are comprised of
competitive products to help satisfy differing needs. The Employer selects the plan and the employees choose
whether or not to participate. Participation requirements apply.
Group Voluntary Term Life
Valuable coverage at affordable group rates with optional Accidental Death and Dismemberment coverage.
Group Voluntary STD and LTD
Can replace income if employee is disabled due to injury or sickness. 2-year "own occupation" definition of disability.
Group Voluntary Cancer
Voluntary Cancer and specified disease coverage in a group policy; 29 diseases covered. Minimum group size is 200 employees with
a minimum participation for issue of 50 employees.
Group Voluntary Dental
The covered percentage paid by the Heritage Choice Dental Plan increases the second and third coverage years. Orthodontic
Services and Braces are available after six months of coverage. Credit is given for previous group coverage sponsored by
the same employer. It is not necessary to choose a network dentist.
Group Voluntary Accident
Our group voluntary accident plan provides 24-hour accident
coverage for the employee or their entire family. Benefits
include Accidental Death & Dismemberment, Hospital Confinement,
Dislocation & Fracture, Ambulance Services, and Medical Expenses.
Add optional Disability riders for Accident and Sickness to
the policy, making it an economical way to include disability
coverage.
Group Voluntary Hospital Indemnity Insurance
Referred to as Group SHOP, (Supplemental Health Options Policy),
it's a plan that the employer builds to help satisfy their
employee's needs. The base policy pays predetermined amounts
for covered hospital, outpatient and physician services. The
employer can round out the plan to fit what their employee's
need by adding other benefits that pay predetermined amounts. Available
options include a Diagnostic benefit, a Preventative "Wellness"
benefit, a Prescription Drug benefit, Short Term Disability
(employee only), Term Life Insurance, and/or Dental benefits. The
employee can choose to purchase coverage for themselves or
their entire family.
Group Voluntary Critical Illness
Provides lump sum benefits for heart attack, organ transplant
and other life threatening conditions. Lump sum benefits for
cancer are also available at the group level. Benefit is paid
at diagnosis and is paid regardless of whether treatment is
chosen. Today, advancements in medical technology and early
detection can mean an increase in the chances of surviving
or living with a critical illness for months—even years.
Benefits from critical illness coverage can be used to help
meet expenses which are not normally covered under traditional
health insurance. Four levels of underwriting available, including
guaranteed issue based on group size, participation and benefit
amount selected. 100% of benefits payable in up to 3 benefit
categories. Plan is portable.
